A quote request collects the quantity, organization, contact name, email, phone, and ship-to address. You can also add a PO, a department, a note, and whether a tax-exempt certificate will follow. A contact note collects your name, email, organization, and message.
The form does not collect a card number. We email you a confirmation and keep a copy so we can reply.
We do not sell personal information.
